In 2023, Zanzibar’s health capacities were evaluated, revealing a need for improved detection and surveillance of antimicrobial resistance (AMR). In response, we established a microbiology lab at Makunduchi Health Centre, in partnership with the Bernhard-Nocht Institute for Tropical Medicine (BNITM). This lab, the only one in Zanzibar offering Antimicrobial Susceptibility Testing (AST), plays a critical role in monitoring AMR.

 

To support this effort, we’ve developed a sample referral system with the Zanzibar Health Research Institute, ensuring samples from across Zanzibar are tested. The data collected will help to:

  • Create local treatment guidelines
  • Inform healthcare decisions
  • Support national AMR strategies.
Our team of dedicated microbiologists is continuously enhancing their skills through collaborations with BNITM and the National Institute for Medical Research in Tanzania.
  • 10 facilities conducted antimicrobial resistance (AMR) surveillance.

  • 2,113 samples processed (17.1% blood and 31.6% urine samples positive).

  • 1,200 participants reached in AMR awareness study across Unguja (health workers, general population, farmers).

HIPZ also supports research and behaviour change initiatives to address antibiotic misuse.

A Knowledge, Attitude and Practice (KAP) study reached 1,200 participants across Unguja — including health workers, the general population, and farmers.

Findings showed knowledge gaps among non-healthcare workers and farmers, particularly around antibiotic use in animals, and confirmed that access to antibiotics remains very easy.

This evidence is guiding the development of Zanzibar’s National Behaviour Change Communication Strategy on AMR, contributing to a more informed and responsible approach to antibiotic use across the islands.
